A few simple steps can help your appointment move efficiently.

Hydrate Normally

A urine specimen is required. The routine DOT urinalysis is not a drug screen.

Limit Caffeine & Nicotine

Avoid them before the appointment when possible because they may temporarily increase blood pressure.

Take Medications as Directed

Do not stop a prescribed medication solely for the examination unless your treating clinician directs you to do so.

How long does the appointment take?

Are same-day appointments available?

Same-day appointments may be available depending on the schedule. We do not accept walk-ins.

Is the urine test a drug test?

No. The required urinalysis is part of the medical examination and is not a drug screen.

What should I bring if I use CPAP?

Bring your most recent 90-day CPAP compliance report.

What should I bring if I use insulin?

Bring a completed MCSA-5870 from the clinician who manages and prescribes your insulin. View the FMCSA form.

Will every driver receive a two-year medical certificate?

No. Two years is the maximum certification period. The actual period depends on the driver’s history, examination findings and whether a condition requires closer monitoring.

What happens if additional information is needed?

Dr. Ezell may request records, testing or information from a treating clinician or specialist before completing the certification decision.
